The regular meeting of the City of Chino Hills City Council for January 25, 2022, will be conducted in-person and remotely through Zoom temporarily in response to the surge of Omicron cases and the City Council may make a finding that certain conditions exist to continue conducting public meetings via teleconferencing pursuant to Government Code Section 54953(e).

CONSENT CALENDAR (12 ITEMS) - All matters listed on the Consent Calendar are considered routine by the City Council and may be enacted by one motion in the form listed below. There will be no separate discussion of these items unless, before the City Council votes on the motion to adopt, Members of the City Council or staff request the matter to be removed from the Consent Calendar for separate action. Removed consent items will be discussed immediately after the adoption of the balance of the Consent Calendar.
 
9.   Approve January 11, 2022, City Council Meeting Minutes
 
10.   Ratify payments over $25,000 totaling $6,174,329.99 for month ended December 31, 2021
 
11.   Receive and file Investment Report for month ended December 31, 2021
 
12.   Authorize execution of Amendment No. 5 to Agreement No. A02-80 with T-Mobile West, LLC, to amend sections relating to City Manager approvals; cost reimbursements; noticing; modify existing telecommunications equipment; and add new access restrictions and requirements for cell facility at 3982 Eucalyptus Avenue (Monopine - Booster Station B-9)
 
13.   Authorize execution of Assignment and Assumption Agreement with Sprint PCS Assets, LLC and VB Run, LLC to transfer all interest, rights, burdens, and obligations of Sprint under License Agreement No. A98-57 for wireless communications site at 15163 Aqueduct Lane
 
14.   Adopt budget amendment resolution for Fiscal Year 2021-22 increasing General Fund budget by $15,737 for General Plan Update and authorize execution of five-year agreement with EcoTierra Consulting, Inc., in amount not-to-exceed $267,940 for preparation of technical studies and Environmental Impact Report for General Plan Update
 
15.   Authorize execution of Subscriber Service Agreement with Online Utility Exchange to provide ongoing credit check services to determine if/when security deposits are collected for utility accounts
 
16.   Adopt budget amendment resolution for Fiscal Year 2021-22 increasing Water Fund budget in amount of $6,315,000 to pay off 2012 Water Revenue Bonds
 
17.   Adopt budget amendment resolution for Fiscal Year 2021-22 increasing budget in various funds in amount of $295,612.05; award contract to Gentry Brothers, Inc., in amount of $866,872.80; and authorize City Manager to approve cumulative change orders up to ten percent ($86,687.28) of awarded contract amount for Los Serranos Safe Routes to School (East) Project
 
18.   Accept Glen Ridge Drive Pavement Rehabilitation project by COPP Contracting, Inc., as complete and authorize City Clerk to record Notice of Completion
 
19.   Adopt budget amendment resolution for Fiscal Year 2021-22 increasing Community Facilities District 2 fund budget in amount of $28,000; approve Change Order No. 1 to Agreement No. A2021-097 in amount of $33,767.24 with COPP Contracting, Inc.; accept Founders Drive Pavement Rehabilitation Project as complete; and authorize City Clerk to record Notice of Completion
 
20.   Find that Carbon Canyon Invasive Palm Tree Injection Project is exempt from review under California Environmental Quality Act; waive competitive bidding procedures under Chino Hills Municipal Code Section 3.18.050.I; and authorize execution of one-year agreement with Santa Ana Watershed Association (SAWA) not-to-exceed $48,088 for palm tree injection treatment in Carbon Canyon area from border of Brea to Chino Hills Parkway
