This agenda contains a brief general description of each item to be considered. Except as otherwise provided by law, no action shall be taken on any item not appearing on the agenda unless the City Council makes a determination that an emergency exists or that a need to take immediate action on the item came to the attention of the City subsequent to the posting of the agenda. The City Clerk has on file copies of written documentation relating to each item of business on this Agenda available for public inspection in the Office of the City Clerk, in the public binder located at the entrance to the Council Chambers, and on the City's website at www.chinohills.org  while the meeting is in session. Materials related to an item on this Agenda submitted to the Council after distribution of the agenda packet are available for public inspection in the Office of the City Clerk at 14000 City Center Drive, Chino Hills, CA during normal business hours.

CONSENT CALENDAR (15 ITEMS) - All matters listed on the Consent Calendar are considered routine by the City Council and may be enacted by one motion in the form listed below. There will be no separate discussion of these items unless, before the City Council votes on the motion to adopt, Members of the City Council or staff request the matter to be removed from the Consent Calendar for separate action. Removed consent items will be discussed immediately after the adoption of the balance of the Consent Calendar.
 
11.   Approve June 13, 2017 City Council Meeting Minutes
 
12.   Approve Warrant Registers for period of June 1 through 14, 2017 in amount of $2,506,942.60
 
13.   Receive and file City Official Reports pursuant to Travel, Training and Meetings Reimbursement Policy for period of May 23 through June 13, 2017
 
14.   Amendment No. 1 to Agreement No. A08-66 with Southern California Edison Company and New Cingular Wireless PCS, LLC, to allow minor modifications to existing facility
 
15.   Authorize execution of three-year Agreement with Imperial Paving Company, Inc., with two one-year renewal options in annual amount not-to-exceed $250,000 with total contract amount not-to-exceed $1,250,000 to provide on-call asphalt repair services
 
16.   Authorize execution of three-year Agreement with ThyssenKrupp Elevator with two one-year renewal options in annual amount not-to-exceed $23,200 with total contract amount not-to-exceed $116,000 for elevator maintenance, testing, and repair services
 
17.   Authorize execution of three-year Agreement with Cardlock Fuels Systems, Inc., dba SC Fuels with two one-year renewal options in annual amount not-to-exceed $200,000 with total contract amount not-to-exceed $1,000,000 for purchase of fuel for City vehicles and equipment
 
18.   Authorize execution of Amendment to Cooperative Agreement No. A2017-077 (16-1001387) with San Bernardino County Transportation Authority to increase maximum reimbursement amount to $68,000 for the San Bernardino Valley Coordinated Traffic Signal System
 
19.   Adopt resolution approving design and plans for construction of English Creek/Channel Habitat Mitigation Project, and authorize Staff to solicit bids for construction
 
20.   Approve Change Order No. 1 to Agreement No. A2017-057 with American Asphalt South, Inc., for credit in amount of ($6,917.60), accept Rehabilitation of Grand Avenue from Chino Hills Parkway to West City Limit Project as complete, and authorize City Clerk to record Notice of Completion
 
21.   Approve appropriations budget amendment in amount of $15,000 from Measure I Fund balance to Los Serranos Infrastructure Improvements Safe Routes to Schools Phase 2 Sidewalk Construction project, Change Order No. 1 to Agreement No. A2016-206 with VT Electric, Inc., in amount of $26,273.75, accept installation of Street Lighting as complete, and authorize City Clerk to record Notice of Completion
 
22.   Authorize Cost Sharing Agreement with Chino Basin Watermaster to establish framework for implementation and funding of Chino Hills Aquifer Storage and Recovery Pilot Project
 
23.   Adopt resolution adopting amended Measure I Five-Year Capital Improvement Program
 
24.   Authorize Purchase Order with EvoQua Water Technologies in amount not-to-exceed $35,000 for purchase of Bioxide for use at sewer lift stations
 
25.   Adopt resolution authorizing annual levy of special taxes for Vila Borba, Community Facilities District No. 2015-1, for Fiscal Year 2017-18

26.   Authorize execution of Amendment No. 1 to Agreement No. A2014-37 to increase billing rates from $190 to $205 per hour for City Attorney services
 
27.   Introduce an Ordinance entitled: An Ordinance of the City Council of the City of Chino Hills Adding Chapter 1.14 to the Chino Hills Municipal Code Changing the City's Electoral System for its City Council Members from At-Large to District-Based Elections, Establishing District Boundaries, and Scheduling Elections Within the Districts, and determine it is exempt from review under the California Environmental Quality Act for first reading by title only - Waive further reading
 
PLANNING COMMISSION MATTERS - This portion of the City Council Agenda is for matters from the June 20, 2017 Planning Commission Meeting. No action is required unless two members of the City Council wish to request a review of the matter, in accordance with Section 16.58.070 of the Chino Hills Municipal Code. Expiration of the public appeal period on Planning Commission Item 28 is July 5, 2017, 5:30 p.m.
 
28.   Custom Home Design Review No. 438 - 16872 Sumach Lane, Mathy Fredeluces, representing owner Omar Dalati: Adopted a Resolution for the construction of a 2,660-square foot, two-story single-family detached home with an attached 515-square foot two-car garage and determining that the project is exempt from review under the California Environmental Quality Act based on the findings of facts and subject to Conditions of Approval

29.   Adopt Resolution modifying Master Schedule of Fees and determine the adoption of the fees is exempt from review under the California Environmental Quality Act
 
30.   Adopt resolution confirming the Engineer's Report for Chino Hills Landscape and Lighting District No. 1 and ordering the levy and collection of assessments
 
31.   Adopt resolution confirming the Engineer's Report for Chino Hills Los Serranos Lighting Maintenance District and ordering the levy and collection of assessments
 
32.   Adopt resolutions confirming the Engineer's Reports for Chino Hills Special Maintenance Areas 1 and 2 and ordering the levy and collection of assessments
